Abstract/Notes Any organization or company must always be dealing with risk. Risk management consists of several steps, one of them is risk assessment. In this study, risk assessment models is developed with PLTSa Kota Surabaya as a research object. The aim of this study is to develop a risk assessment models by integrating the Delphi method, Fuzzy Cognitive Maps (FCM), and Fuzzy Synthetic Evaluation (FSE) in real cases, and proposing risk mitigation steps by considering the relationship of the risks. The Delphi method is used to identify risk. FCM was used to identify the relationship among risks and the relationship between risks with the objectives of PLTSa. FSE was used to assess the level of risks in order to find risks in the critical category. Delphi method identified 15 risks as a result and reached the consensus based on statistical analysis. FCM generated positive value for all relationship among identified risks and negative value for all relationship between identified risks with the objectives of PLTSa. Meanwhile, FSE method obtained six types of critical risk, RC1 = 3,082, RC2 = 3,041, RC3 = 2,954, RC4 = 2.915, RC5 = 2,872, and RC6 = 2.828). Mitigation proposal consist of three parts, which are maintenance, human resources, and fuels quality.
